- Accomplished Supplier Accreditation Questionnaire signed by President/General Manager
- Notarized Certification on Former Maynilad Employees
- SEC Registration or DTI Registration
- BIR Registration (Form 2303)
- Valid Mayor’s Permit
- Latest Audited Financial Statements
- Bank Certificate indicating balance (for new companies, if Financial Statement is unavailable)
- Notarized Anti-Fraud Declaration
- DOLE 18-A/174 Certificate
- Accomplished Supplier Accreditation Questionnaire signed by President/General Manager
- Company Registration
- Latest Audited Financial Statements/Bank Certificate indicating balance (for new companies)

REQUEST FOR PRODUCT PRESENTATION
Any supplier may request for a Product Presentation or Demonstration. Suppliers must submit a technical proposal or brochures of the products to [email protected].
Presentations are scheduled depending on the availability of participants and venue. Suppliers are advised to send their requests at least two weeks in advance to ensure availability.ADVISORIES AND UPDATES
